Notice Regarding Independent Mental Health Professionals and Other Professionals
At the time of this Notice, Triangle Therapist, LLC operates primarily as a technology platform and professional directory designed to assist individuals in locating independent licensed mental health professionals throughout North Carolina.
Unless expressly identified otherwise, each therapist, psychologist, counselor, clinical social worker, marriage and family therapist, psychiatric provider, behavior therapist, coach, or other healthcare professional listed through the Triangle Therapist directory is an independent practitioner who is solely responsible for the professional services they provide.
Triangle Therapist, LLC does not supervise, direct, control, or make clinical decisions on behalf of independent providers. Each provider independently determines whether to accept a prospective client, establishes the therapeutic relationship, maintains the clinical record, develops treatment plans, documents services, bills for professional services where applicable, and remains responsible for complying with all applicable federal and state laws governing the practice of their profession.
Accordingly, any Protected Health Information (“PHI”) created, maintained, or received by an independent provider in connection with the delivery of healthcare services is the responsibility of that provider, unless Triangle Therapist, LLC is separately acting on behalf of that provider pursuant to a written agreement or applicable law.
If you choose to receive services from a provider listed on the Triangle Therapist directory, your healthcare relationship exists directly between you and that provider. The provider’s own Notice of Privacy Practices, informed consent documents, professional policies, and applicable legal obligations govern the collection, use, disclosure, storage, and protection of your Protected Health Information.
Triangle Therapist, LLC may collect limited personal information necessary to operate its directory and facilitate communications, such as your name, email address, telephone number, appointment requests, or messages voluntarily submitted through the platform. Information collected for these administrative purposes is governed by Triangle Therapist’s Privacy Policy rather than this Notice of Privacy Practices.
Should Triangle Therapist, LLC expand in the future to provide direct clinical services through licensed mental health professionals employed by or practicing on behalf of Triangle Therapist, LLC, this Notice may be revised to describe the privacy practices applicable to those clinical services. At that time, patients receiving care directly from Triangle Therapist, LLC would receive an updated Notice of Privacy Practices explaining how their Protected Health Information may be used and disclosed in accordance with applicable federal and state law.
Nothing in this Notice alters the independent legal, ethical, licensing, or professional responsibilities of any therapist listed within the Triangle Therapist directory. Each independent provider remains solely responsible for maintaining the confidentiality of patient records, complying with applicable privacy laws, safeguarding Protected Health Information, fulfilling professional recordkeeping obligations, and meeting all standards established by their respective licensing boards and governing authorities.
