Every worksheet here connects to a point on the map — the entry that makes it work. Use them between sessions, bring a printed copy to one, or share them with someone you care about. Nothing you type is saved or sent anywhere.
Point 1 · Through a thought
Dysfunctional Thought Record
The core CBT worksheet. Catch the automatic thought, rate how much you believe it, name the cognitive distortion, and write a fairer draft. Seven columns, five rows, printable as a landscape PDF. Most people discover their two or three favorite distortions within a few entries.
Point 3 · Through a feeling
Interactive Feelings Wheel
Seven core emotions branching into 82 specific feelings across three rings. Tap any feeling to light up its full ancestry — from “Angry” down to “Resentful” — and read the path in the center. The vocabulary that makes a thought record, a repair conversation, or a therapy session actually work.
Point 5 · Through your story
Family Genogram
A three-generation family map where every person is a Triangle mark — the Center, the observing self. Name each person, add one word for them, and tap any couple’s line to mark how that bond actually was. Patterns that travel down family lines become visible on one page. Prints in landscape.
Point 6 · Through your relationships
The Repair Path
The Gottmans’ “Aftermath of a Fight” exercise as a five-step interactive worksheet: feelings chips, realities in your own words, triggers, responsibility starters, and a constructive plan. Couples who last don’t fight less — they repair better. Fill it together after you’ve both cooled down. Prints to two clean pages.
Point 6 · Through your relationships
Family-of-Origin Scale
Forty statements about the family you grew up in, drawn from a classic instrument in family therapy research. Your answers are scored instantly across the ten qualities of a healthy family — five that build autonomy, like clarity of expression and personal responsibility, and five that build intimacy, like warmth, empathy, and trust. You get a total, two subtotals, and a bar for every quality, so you can see exactly where your family was strong and where things were harder. Pairs naturally with the genogram: one maps the people, the other measures the climate.

Why free therapy worksheets on a therapist directory?
Every worksheet here connects to one of ten points on our map of modern therapy — the entry that makes it work. The Thought Record enters through a thought. The Feelings Wheel enters through a feeling. The Genogram enters through your story. The Repair Path and the Family-of-Origin Scale enter through your relationships.
Understanding why a worksheet works is what turns a form into an insight. That’s why each one links to the full guide for its therapy family — so you can go as deep as you want.
A note on what these worksheets are and aren’t. These are psychoeducational resources — things to help you understand yourself, prepare for a session, or practice a skill between appointments. They are not therapy and not a substitute for working with a licensed clinician.
- Use them between sessions to deepen the work
- Bring printed versions to your therapist — they will use them
- Share them with a partner, family member, or trusted friend
- Nothing you type is saved or sent anywhere
The worksheets help you understand the map. A therapist helps you move through it.
